  2. Click the button that says “Sign Up.”
  3. Put your name in the fields that are there.
  4. Entering your gender is optional.
  5. Type the patient’s Date of Birth into the fields provided.
  6. Fill in your address in the boxes that are there.
  7. Type a phone number in the field that’s there.
  8. Make a password and type it in the spaces that are there.
  9. In the space provided, type the last four digits of the patient’s Social Security Number.
  10. Under “Authorization,” click the box that says “I have read, understand, and agree to the Web Privacy Statement and Terms of Use.”
  11. Click on the button that says “CREATE ACCOUNT.”
  12. Go on to the next step and give all the information needed to finish registering for a Patient online account. Check that all of your personal information is correct before submitting.

  3. Again, click the “Sign In” button.
  4. In the spaces provided, type in your email address and password.
