- Fill out the fields with your Activation Code.
- Type the last four digits of your Social Security Number in the box that says “Last Four Digits.”
- Enter your birth date in the spaces provided.
- Click the button that says “Next.”
- Go on to the next step and give all the information needed to finish signing up for a new online account.

- If you were not given an activation code or don’t have one, you will need to apply for one in person at the Release of Information office at: Providence Healthcare Network, 6901 Medical Parkway, Waco, TX.
- Bring a valid government-issued ID with you if you want to ask for a MyChart account.
- Steps if you are a CURRENT patient who wants to sign in to your account online:
- Visit: https://mychart.providence-waco.org on your device.
- In the spaces provided, type in your username and password.
- Click “SIGN IN” and you can start taking care of your online account.
