  2. If you click “SIGN UP NOW,” you’ll be sent here: https://mychart.providence-waco.org/MyChart/accesscheck.asp.
  3. Fill out the fields with your Activation Code.
  4. Type the last four digits of your Social Security Number in the box that says “Last Four Digits.”
  5. Enter your birth date in the spaces provided.
  6. Click the button that says “Next.”
  7. Go on to the next step and give all the information needed to finish signing up for a new online account.

  1. If you were not given an activation code or don’t have one, you will need to apply for one in person at the Release of Information office at: Providence Healthcare Network, 6901 Medical Parkway, Waco, TX.
  2. Bring a valid government-issued ID with you if you want to ask for a MyChart account.
  3. Steps if you are a CURRENT patient who wants to sign in to your account online:
